Understand and better manage your asthma, diabetes, depression, cancer or other chronic health condition with the help of a qualified nurse health coach.
Administered by Active Health Management, this program lets you schedule phone consultations with a nurse health coach who will:
- Help you develop a personalized strategy for following your doctor's treatment plan
- Recommend behavioral changes for improving your condition
- Offer ongoing guidance and motivation to help you monitor and improve your health and well-being
A nurse health coach from Active Health Management may call to invite you to join the program.
To get an early start, if you or your covered dependents have a chronic health condition, you can contact Active Health Management at any time.
Confidentiality. By law, information you share with your nurse health coach is strictly confidential and will never be shared with Compass Group.
